Mason Hazlewood, infant son of Donnie and Kari Ann (Wood) Hazlewood of Ponca City died Saturday, May 23, 2015, in the Ponca City Medical Center.
He is survived by his parents of Ponca City; maternal grandparents, Don and Cathy Wood of Blackwell, Oklahoma; paternal grandparents, Terry and Nancy Silkey of Ponca City, Oklahoma, and Don and Marguerite Hazlewood of Eufaula, Oklahoma; paternal great grandfather, Don Hazlewood of Henderson, Texas; paternal great grandmother, Betty Williams of Ponca City, Oklahoma; and aunt, Beth Wood of Blackwell, Oklahoma.
Private services will be held.
A memorial Has been established with Longwood Baptist Church c/o Roberts and Son Funeral Home, 120 West Padon, Blackwell, Oklahoma 74631.
